Youth Participation Waiver

VillageOS Youth Participation Waiver, Parental Consent, and Release

In consideration of the minor Participant being permitted to participate in activities organized, sponsored, or hosted by VillageOS — including farm work, gardening, livestock interaction, community events, educational programs, and on-site visits ("Activities") — I, the undersigned Parent or Legal Guardian, acknowledge and agree to the terms below on behalf of myself and the minor Participant.

1. Parental Authority. I represent that I am the parent or legal guardian of the minor Participant identified above and that I have full legal authority to execute this waiver and provide consent on the minor's behalf.

2. Assumption of Risk. I understand that the Activities involve inherent risks of injury, illness, or death, including but not limited to: contact with tools, machinery, animals, plants, allergens, and biological agents; slips, trips, and falls; exposure to weather, terrain, and water; physical exertion; and the actions of other participants. I voluntarily accept these risks on behalf of the minor.

3. Supervision. I understand that while VillageOS staff will provide reasonable supervision appropriate to the Activity, supervision is not constant or one-to-one. I authorize the minor to participate under group supervision.

4. Release of Liability. I voluntarily release, waive, and covenant not to sue VillageOS, its operators, owners, managers, directors, officers, employees, contractors, volunteers, landowners, and representatives ("Released Parties") from any and all liability arising from any loss, damage, illness, or injury sustained by the minor during the Activities, whether caused by negligence of the Released Parties or otherwise.

5. Indemnification. I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or costs, including legal fees, incurred due to the minor's participation.

6. Medical Authorization. I authorize VillageOS and its representatives to seek, consent to, and provide reasonable emergency medical care, transportation, or treatment for the minor if I cannot be reached, and I accept financial responsibility for any treatment received.

7. Code of Conduct. I agree that the minor will follow all rules, instructions, and safety guidelines provided by Village Managers, and I understand the minor may be removed from Activities without refund for unsafe or disrespectful behaviour.

8. Media Release. Subject to the media consent option selected below, I grant VillageOS permission to use photographs, video, audio, or other media of the minor captured during the Activities for educational, promotional, and operational purposes.

9. Severability and Governing Law. This waiver is governed by the laws of the Province of Ontario, Canada. If any provision is held invalid, the remaining provisions remain in full force.

10. Acknowledgement. I have read this waiver in full, understand its terms — including that I am giving up substantial legal rights on behalf of myself and the minor Participant — and acknowledge I am signing it freely and voluntarily.
